Download and complete our Medications and Allergies form.

The Medications and Allergies form is available for download in the PDF link below this paragraph. This may include but is not limited to prescription medications, over the counter medications, vitamins, or herbal supplements.


Bring all previous available medical records to your appointment.

Examples of medical records may include but are not limited to office notes, doctor's remarks, emergency room notes, or urgent care records.

Your doctor's office(s) or testing facilities may request a release from you before supplying records. Some offices may fax record for you; the fax number for our office is (919) 479 - 4116. Please do not fax packets of records that exceed 40 pages. If you have records in excess of 40 pages, bring them with you on the day or your appointment or have them mailed to the  office. It is a good idea to begin gathering records as soon as possible, as it may take a week or more for your doctor to compile this information. 


Bring all of your testing reports and imaging results

Examples of testing reports and imaging results may include but are not limited to MRIs, X-rays, CT scans, or Myelograms.


Bring your current insurance card(s) and a specialist co-pay

The specialist co-pay should correspond to your card. We accept many insurance policies including but not limited to the following:

  • BCBS



  • United Healthcare

  • Medicare
